We provide highly personalised care to residents of care homes aligned to the PCN. Each care home has a nominated lead GP who makes weekly contact with the home. The lead GPs have worked together to standardise care plans across the PCN, ensuring that all residents receive the same high quality care which is tailored to their individual needs and concerns. The residents also benefit from Structured Medication Reviews (SMRs) with a clinical pharmacist to reduce overprescribing and ensure that a resident's medication is working well for them.
Care home Residents take priority in the covid-19 vaccination campaigns, they are visited by the PCN vaccination team.
