Tiverton PCN brings together a range of health and social care professionals to provide personalised and preventative care for our local community.
Primary Care Networks (PCNs) are organisations consisting of neighbouring GP practices, working closely with other community healthcare providers to improve the health and wellbeing of patients. Based on GP practice lists and generally serving populations of 30,000-50,000, PCNs are small enough to provide the personal care valued by both patients and practice staff but large enough to have economies of scale through better collaboration between practices and others in the local health and social care system.
October 1st 2024 - Tiverton PCN are really pleased to announce we have expanded, Amicus Health have merged to integrate with Mid Devon Medical Practice at Witheridge with their satellite sites at Morchard Bishop and Cheriton Fitzpaine, this increases our patient population from 32,000 to 37,500. Sharing resources, team members, knowledge and expertise to serve a wider community.
